Once you have completed and sent the Enrollment Worksheet, we will prepare a plan analysis for you using the medications, strengths, and quantities you have entered. Please be sure to enter your medication information as accurately as possible. If more than one person in your household needs an analysis, please enter them on separate worksheets.
If you have entered an email address, we will send the plan comparison and your invoice ($19.95-$34.95 see worksheet for details) to your email account. If you would like, the packet can also be mailed to your home.
We will then contact you to be sure that you have received your packet and to answer any questions that you may have.
If you wish to enroll in one of our plans, we can send the appropriate enrollment kit via email (or U.S. mail). It is that simple to be sure your are in the right plan!
