I see a monthly premium for Accidental Death & Dismemberment AD&D being directly deducted from my checking account. I don't think that I ever signed up for such insurance coverage! Where is this deduction coming from?

You probably have the wrong insurance provider.

We do offer Accidental  Death & Dismemberment (AD&D) coverage through Petersen International Underwriting / Lloyds of London.

However, we do not market an Accidental Death & Dismemberment (AD&D) product that deducts a payment from your banking account.  The premiums for our AD&D coverage are paid before an AD&D  policy is issued.

Please note, you are not alone.  Such questions like this about  AD&D payments are rather common.  We have found over the years that many people signed up for an AD&D policy through their employer and are having the monthly premiums directly deducted from their checking or savings account (or they may have a monthly credit card charge).

What can you do?

(1) Please check with your employer's Human Resources or Benefits departments (to confirm that you do, in fact, have an Accidental Death & Dismemberment  policy?) or

(2) Call the telephone number that appears in your bank statement and ask the insurance company about the charges or your policy or

(3) Contact  your bank (or credit card company) directly and ask them about the details surrounding this payment.

Hopefully, one of these three methods should help you to clarify your AD&D coverage or payments.
