| How do I cancell (dis-enroll) from my plan? |

If you are enrolling in a new plan, you simply need to complete the enrollment form for the new plan. When the insurance company sends your information to Medicare, Medicare will notify your original insurance carrier that you have chosen a new plan and you will be dis-enrolled. This change may take a bit of time to be processed.
If you would like to simply dis-enroll or want to notify the insurance carrier that you are cancelling your plan, and you have a direct telephone number for the Billing & Enrollment Department, give them a call.
For example, Humana's Billing & Enrollment Department can be reached at 1-800-992-2551.
If you do not have a telephone number for Billing & Enrollment, simply call the telephone number on the back of your Medicare Part D Membership card.
